This policy applies to Sal Aesthetic Clinic, its website and its clinic rooms in Bankstown, Campsie and Campbelltown. It is written to meet the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
what we collect
Through this website: your name, phone number, email address, the concern you selected, your preferred location, and anything you choose to write in the message field. Standard technical information such as your browser type and the pages you viewed is also collected by the hosting platform.
In the clinic: health information, including your medical history, medications, allergies, previous procedures, clinical notes, consent records and any clinical photographs taken. Health information is sensitive information and is treated as such.
why we collect it
To respond to your enquiry, to book and manage your appointments, to provide clinical care safely, to keep the health records the law requires us to keep, and to contact you about your own care.
We do not sell personal information. We do not add you to a marketing list because you sent an enquiry.
who else sees it
Enquiries submitted through this website are delivered by email through Resend, our email provider. Appointment bookings are handled through Timely, our booking system. Both providers process information on our behalf.
Clinical information is disclosed to another health practitioner only where it is necessary for your care, or where we are required or permitted by law to do so.
clinical photographs
Clinical photographs may be taken as part of your record. They are stored securely and are used for your clinical care.
Images of patients are not used in advertising by this clinic. If that ever changed, it would require separate written consent from you, given independently of your consent to any procedure, and you would be free to refuse or to withdraw that consent at any time.
how long we keep it
Health records are retained for the period required by New South Wales law, which is generally seven years from the date of the last entry for an adult, and until the age of 25 for a person under 18 at the time of the record.
Website enquiries that do not become appointments are kept only as long as needed to deal with them.
access and correction
You can ask for access to the personal information we hold about you, and you can ask us to correct it. Email salaestheticclinic@gmail.com or ring the clinic. We may need to verify your identity first.
If you are not satisfied with how we have handled your information, you can complain to us directly, and you can escalate to the Office of the Australian Information Commissioner at oaic.gov.au.
security
Information is held in secure systems with access limited to the practitioner and authorised staff. No system is perfect, and if a data breach occurs that is likely to cause you serious harm, you will be notified in line with the Notifiable Data Breaches scheme.
